Dr. Wheeler’s View on Men’s Health

What men's health advice do you have for UFM Underwear Supporters? Be educated. Patients have a choice in diagnostics and they have a right to make an informed decision about their treatment. What the doctor says may not be in your best interest. How important is it for men to talk about their health? We come from an era of ignorance, which is why it is very important for men to talk to other men about men's health issues and concerns. Men with prostate cancer especially need to talk to their sons. Otherwise, family issues become a neighborhood issue, which becomes a community issue, which becomes a state issue and so on. Prostate cancer is a disease that does not discriminate, wreaking havoc on the lives of young men, older men, wealthy men, poor men, wise men, leaders, followers, educated men, under-educated men, accomplished men, underachieving men, black men, white men, red men, brown men; men from all over the world. Regardless of their educational background, when it comes to prostate cancer, all men are equally ignorant. How important is it to get a second opinion? A second opinion is really just another opinion. The biggest area of contention in the field of Urology is whether or not to do a biopsy.  It is important for every patient to know what to expect when they agree to a biopsy. The side effects of a biopsy can include, bleeding, erectile dysfunction, incontinence, sepsis, hospitalization and death. At our practice, we are using imaging systems to show the way. Talk to you doctor about what treatments are right for you and don’t be afraid to ask another doctor for their perspective. When should men get a prostate exam?

Make no mistake about it, prostate disease in general and prostate cancer specifically, is a disease for all ages, with over 250,000 men diagnosed with prostate cancer every year. The disease has no boundaries, no conscience, and will strike most times with no warning other than an elevated PSA (prostate specific antigen). Your only defense is to be educated on the disease, get motivated and become proactive while optimally maintaining a PSA of less than 0.7 ng/ml. You don’t have to look sick to be sick. We all know we need to pay attention and get checked. I recommend that men start getting checked in their 30s. If everything looks good, I recommend rechecking every 1-2 years until 40 when a PSA should be done every year on your birthday. What are the latest prostate cancer treatments? Clearly, I consider myself a pioneer. In Urology, I have been introduced to all the prostate cancer treatment options and I chose the best; HIFU (high intensity focused ultrasound). HIFU is a minimally invasive procedure, which offers dramatically reduced side effects when compared to Radiation or Radical Prostatectomy. Over the last 20 years, nearly 40,000 men have had their prostate treated with HIFU. HIFU eradicates prostate cancer through the precise focusing of sound waves to a targeted focal point of disease. A biopsy only offers 20-30% accuracy in a randomized sampling. If you stick a needle into a prostate cancer, you knowingly spread it (Reference: Journal of Urology, Vol.168, 2120, November 2002). Using 3T Multi-parametric MRI-guided diagnostic exam will allow for a non-invasive diagnosis of prostate cancer with 98-99% accuracy. When HIFU is performed the “Wheeler way” cure rates are greater than 99% assuming the highest PSA value was 8.5 ng/ml or less (regardless of the Gleason score). A 3T MP-MRI scan puts us in a position to know more, do more and cure more men with a non-surgical innovation called  HIFU, the choice of the educated man. Since all patients are not candidates for HIFU, it is important to discuss the options with your healthcare professional or call us for a free consultation (877-766-8400). How do you define a HIFU expert? A HIFU expert is someone who has excellent results in HIFU with outcome data in excess of 90% and a non-existent PSA (less than 0.2 ng/ml), statistically the same as zero while treating more than 100 patients. What role does nutrition play in Urology Health? “You are what you eat” is Chapter 5 of my book speaking to the Modified Mediterranean Diet and nutritionals that work. In Chapter 6 of my book, Men at Risk: The Dirty Little Secret, Prostate Biopsies Really Do Spread Prostate Cancer Cells, I discuss the perils of biopsy. While the book is a must read for all men aged 35 years or older, it is well known that 40-60% of men that have radiation treatments or Radical Prostatectomy for prostate cancer failed to be cured at 7 to 10 years. This can be attributed to many factors including your diet and environment factors such as pollution, radiation, cigarette smoke and herbicides or the wrong treatment choice. It is important for everyone to eat and live healthy, especially men who are susceptible to prostate cancer. That would be all men if we live to be 100 years old.
